Person with Disability (PWD) ID
Apply for or replace a PWD Identification Card at the Dipolog City Social Welfare and Development Office (CSWDO).
Requirements for the ID
- Birth Certificate or Baptismal Certificate
- Income Tax Return (if any)
- Certificate of Residency from the Barangay Captain
- Two (2) copies of 1x1 ID picture
- Accomplished membership information sheet (c/o the PWD Center)

- Affidavit of Loss
- Barangay or police blotter
