Birth Registration (Timely and Delayed)
Register a birth that occurred in Dipolog City with the Office of the City Civil Registrar, on time (within 30 days) or as a delayed registration.
Timely Registration (within 30 days): Legitimate Child
- Duly accomplished Certificate of Live Birth (COLB), 4 original copies, with Data Sheet (1 copy), from the hospital, clinic, or trained hilot/midwife who assisted the birth
- Marriage Contract of the parents, 1 photocopy (from PSA or the Local Civil Registrar where the marriage took place)
Timely Registration: Child Not Acknowledged by the Father (uses mother's surname)
- Duly accomplished Certificate of Live Birth (COLB), 4 original copies, with Data Sheet (1 copy), from the hospital, clinic, or trained hilot/midwife who assisted the birth
Timely or Delayed: Child Acknowledged by the Father (uses father's surname)
- Duly accomplished Certificate of Live Birth (COLB), 4 original copies, with Data Sheet (1 copy), from the hospital, clinic, or trained hilot/midwife who assisted the birth
- Valid identification documents of the parents, 1 copy per parent
- Community Tax Certificates (cedula) of the parents, 1 original or photocopy per parent (Barangay Hall or City Hall)
Delayed Registration (beyond 30 days): Legitimate Child
- Duly accomplished Certificate of Live Birth (COLB), 4 original copies, with Data Sheet (1 copy), from the hospital, clinic, or trained hilot/midwife who assisted the birth
- Marriage Contract of the parents, 1 photocopy (from PSA or the Local Civil Registrar where the marriage took place)
- Community Tax Certificates (cedula) of the parents, 1 original or photocopy
Delayed Registration: Child Not Acknowledged by the Father
- Duly accomplished Certificate of Live Birth (COLB), 4 original copies, with Data Sheet (1 copy), from the hospital, clinic, or trained hilot/midwife who assisted the birth
- Community Tax Certificate (cedula) of the parent, 1 original or photocopy

Good to know
- Total processing time per the Charter: 20 minutes (timely), 34 minutes (timely, acknowledged by the father), 33 minutes plus the 10-day posting (delayed), 45 minutes plus the 10-day posting (delayed, acknowledged by the father).
- This registers the birth with the city. Security-paper (PSA) copies are requested separately from the Philippine Statistics Authority.
